We ask that you fill this out to give us a little information about yourself. We do not require a deposit from you to be added to our puppy list. Deposits are due when puppies are born. 2. Your preference of puppy (sex, color) and are you wanting limited or full registration?

3. Do you have a fenced in yard for your new puppy?

4. If purchasing one of our puppies for pet homes, will you have your puppy spayed/neutered at the appropriate age (usually done at 6 months old to 1yr old)?

5. Where will your puppy stay inside/outside?

6. If you are planning on keeping your puppy outside what kind of housing will you provide for your puppy?

7. Please give your veterinarian's name and phone number? 

8. We always want the best for our puppies we sell and never want to see them go to a shelter; are you willing to give the puppy/dog back if you are no longer able to care for him/her?

**Our puppies are microchiped with the Avid euro chip which is readable by other scanner types. We encourage you to register your puppy with Avid so they can help you recover your puppy if ever lost. If you leave us or would like to leave us as a second contact, we are happy to  help in every way to get your puppy back to you safely as quickly as possible.**
